As part of the pilot projects INTEGREO, networks of Belgian professionals and institutions are collaborating to implement integrated care at the local to regional level. The final goal is to improve population health through better integration of care (health and social). Each of the 13 pilots established an action plan in the conceptualization phase of the project with interventions aiming for a series of targets. Targets are delivered care services or sequences of delivered care with low or high health value, such as inappropriate use of the Emergency department. In some cases, these interventions will also allow better use of resources and if possible later on a redistribution of them. The projects established a series of hypotheses that would explain the effect of the planned intervention on the target through a series of mechanisms according to a program theory. In this process, integration management and network governance are the key to successful implementation. The mission of the FAITH inter-university consortium is to carry out an external evaluation of the implementation and to support pilot projects for self-evaluation. In this context, FAITH co-develops with them a dashboard in order to integrate a logic of population health management and to bring elements to reflect on the decision-making process of the governance of the project. This tool is one of the pillars of (scientific) support for projects. The dashboard is developed in stages according to the availability of data and the progress of implementation at project level. This information is related to the interventions carried out and the results observed (population health and consumption of care, costs, patient experience and experience of the professionals involved). The interaction with the projects will allow to tailor the tools to their targets and interventions.
