The co-creation in the design of a dashboard to develop a population health approach initial experience from Integreo program (Belgium)

As part of the pilot projects INTEGREO, networks of Belgian professionals and institutions are collaborating to implement integrated care at the local to regional level. The final goal is to improve population health through better integration of care (health and social).
